💼 About This Role

You'll create and oversee personalized care plans for patients in their homes, coordinating with an interdisciplinary team to promote wellness and prevent re-hospitalization. You'll use your clinical skills for wound care, IV medications, and CHF management while enjoying flexible scheduling and advancement opportunities.

🎯 What You'll Do

• Conduct comprehensive patient assessments and develop care plans.
• Case manage care episodes with interdisciplinary clinical team.
• Administer IV medications and perform wound care, tube feedings.
• Collaborate with physicians and document clinical notes.

📋 Requirements

• Current and unrestricted CA RN license.
• Current BLS/CPR certification.
• Valid CA Driver's License.
• Experience with high acuity patients or home health preferred.

• Experience with IV medication infusions.
• Proficiency in intravenous (IV) therapy.
• Case management fundamentals.
