💼 About This Role

You'll collaborate with an interdisciplinary team to develop and implement nutrition services for seniors in a PACE program. Your work will directly impact participants' health through assessments, counseling, and meal planning. This role stands out because you'll see the direct impact of your work in a holistic care environment.

🎯 What You'll Do

• Conduct nutrition assessments to determine participant needs and goals
• Participate in IDT meetings to develop care plans
• Deliver and document nutrition interventions per physician orders
• Oversee menu planning and meal delivery programs

📋 Requirements

• Registered Dietitian credential
• At least 2 years directing nutrition services in healthcare
• Current BLS and First Aid certification
• At least 1 year experience with frail or elderly population
