Description

In this role, you will provide rehabilitative care to adult and geriatric clients in their homes through home health visits in Northeast Baltimore. You'll develop and implement individualized care plans, perform diagnostic tests, and collaborate with a multi-disciplinary team to promote clients' health and independent living.

Requirements

• Current Maryland Physical Therapist license
• Minimum one year recent work experience as a Physical Therapist
• Graduation from an accredited Physical Therapy program
• Ability to work independently and manage time effectively
• Strong interpersonal, organizational, and problem-solving skills

Responsibilities

• Perform home visits in designated territories
• Conduct diagnostic tests and measurements (e.g., mobility, balance, strength)
• Develop and implement individualized care plans including manual therapy and gait training
• Educate clients and families on rehabilitative care
• Document observations and interventions using electronic medical records
