💼 About This Role

You'll oversee a team of ECM staff to address clients' medical, behavioral, and social needs, ensuring high-quality care management. You'll drive key performance indicators and collaborate with the broader care team to provide client advocacy. This role offers the chance to shape processes in a growth-oriented environment.

🎯 What You'll Do

• Supervise 10-15 direct reports on the ECM team.
• Oversee client outreach, enrollments, and care plans.
• Drive team to meet key performance indicators (KPIs).
• Provide clinical oversight and direct care management as needed.

📋 Requirements

• Active RN license with ability to obtain California licensure.
• 2 years of people management experience.
• 2-3 years of care management experience.
• Willing to work Monday-Friday 8:30am-5:00pm Pacific Time.

✨ Nice to Have

• 4 years of people management experience.
• 1-2 years of post-acute experience.
• Knowledge of lean Six Sigma methodology.
