💼 About This Role

You'll manage a diverse patient panel in a human-centered primary care practice, part of an innovative healthcare model backed by Amazon. You'll conduct in-office and telehealth visits while collaborating with a supportive team to transform healthcare delivery. Ongoing learning through weekly Clinical Rounds and supervision of NP/PA colleagues are key aspects of this role.

🎯 What You'll Do

• Manage a patient panel with acute, chronic, and well visits
• Treat patients in-office and conduct occasional telehealth visits
• Participate in weekly Clinical Rounds and continuous learning
• Supervise one or more NP or PA colleagues

📋 Requirements

• Enrolled in or completed an accredited Family Medicine residency program
• Practiced at least 2 of the last 5 years in outpatient primary care
• Board Certified in Family Medicine or Board Eligible with plans to certify within 1 year
• State licensed in IL before start date
