Description

Provide home health physical therapy services to patients in Barry, Lawrence, and Dade counties. Perform 1:1 direct care, develop care plans, and educate clients and families. Enjoy flexible schedules, hands-on training, and 24/7 clinical support.

Requirements

• Active and current PT license
• One year clinical experience preferred
• Ability to travel locally to cases
• Willingness to train in home health care
• Valid driver's license

Responsibilities

• Make home visits to clients in designated territories
• Perform diagnostic tests and measurements (mobility, strength, balance)
• Implement integumentary protective interventions
• Develop and implement individualized care plans
• Educate clients and family members on rehabilitative care
