Requirements

• Active LPN license in Minnesota
• Graduation from an accredited nursing program
• Commitment to minimum two shifts per month for PRN
• Availability for daytime, overnight, 8-12 hour shifts
• Pass background checks and comply with state mandates

Responsibilities

• Provide reliable, compassionate nursing care to clients in their homes
• Perform 1:1 care for clients of all ages and acuity levels
• Document patient care using electronic charting (Cubhub)
• Collaborate with clinical team for 24/7 support
• Adhere to care plans and state regulations
