Description

You will provide advanced healthcare services in a 100% outpatient primary care clinic, diagnosing illnesses, prescribing medications, and collaborating with interdisciplinary teams to deliver holistic patient-centered care. This role focuses on improving health outcomes in a value-based care model.

Requirements

• Nurse Practitioner and Registered Nurse licenses in Texas
• Master's in nursing required; Doctorate of Nursing preferred
• National board certification as a Nurse Practitioner from an accredited agency
• Two or more years of clinical experience as a nursing practitioner in a physician practice or hospital
• Bilingual in English and Spanish preferred

Responsibilities

• Examine patients, discuss medical history and symptoms, and perform physicals
• Order and interpret diagnostic tests, including x-rays, blood work, and scans
• Prescribe medications and advise patients on treatment plans
• Update patient charts and upload to Electronic Health Record (EHR) software
• Collaborate with a team of nurse practitioners, registered nurses, and doctors
