💼 About This Role

You'll provide psychosocial services to terminally ill patients and their families in a faith-based hospice ministry. You'll conduct initial assessments, coordinate care plans, and serve as a liaison to community resources to ensure holistic support during end-of-life care.

🎯 What You'll Do

• Conduct comprehensive psychosocial and bereavement assessments
• Refer patients/families to community resources and financial assistance
• Provide anticipatory grief counseling to patients and families
• Participate in interdisciplinary team meetings to coordinate care

📋 Requirements

• Master's degree in social work from CSWE-accredited school
• Active Ohio Social Work license
• 1 year healthcare experience with terminal illness population
• Valid driver's license and insured reliable transportation
