Description

You will provide fully remote, video-based comprehensive primary care including preventive care, sick visits, and chronic condition management. Supported by K Health's Clinical AI Platform, you'll manage a dedicated patient panel and participate in population health initiatives.

Requirements

• Board-Certified in Family or Internal Medicine
• 3+ years post-residency work experience
• 2+ years in-person primary care experience
• Experience with virtual visits
• Active CA medical license, IMLC preferred

Responsibilities

• Provide preventative, acute, and chronic care in a fully remote virtual setting
• Manage a dedicated panel of patients as their primary care physician
• Refer patients to specialists or acute care services when clinically indicated
• Work on population health initiatives for risk-attributed patients
• Support patients post-discharge to reduce hospital readmissions
