Description

As a hospice nurse, you will be an integral member of a multi-disciplinary health care team providing palliative care to clients, enabling them to remain at home. You will make home visits, administer medications, perform wound care, monitor conditions, and document doctors' orders.

Requirements

• Current Registered Nurse (RN) license in Pennsylvania
• Minimum one year of recent verifiable clinical (medical/surgical) experience
• Solid computer skills; prior experience with electronic medical records (EMR) preferred
• Ability to travel
• Prior home care and/or hospice experience strongly preferred

Responsibilities

• Making home visits to clients in designated geographic territories
• Administering medication, wound care, treatments, procedures, and comfort care
• Monitoring clients' conditions and reporting changes to Clinical or Client Services Manager
• Following up with and executing doctors' orders and proper documentation
• Performing client assessments as necessary
