You'll serve as Waymark's frontline presence in Kane County, connecting Medicaid patients to social support and resources. Your work will directly improve health outcomes for low-income communities by addressing social determinants like housing and food access. You'll be part of a multidisciplinary care team that includes clinical social workers and pharmacists.

🎯 What You'll Do

• Conduct needs assessments with patients in homes and healthcare facilities.
• Help patients with health-related social issues like homelessness and hunger.
• Build and manage a directory of community resources.
• Document patient encounters in technology systems.

📋 Requirements

• Self-motivated to work independently and manage schedules.
• Computer literate for virtual communication and documentation.
• Reside within a commutable distance of Kane County, Illinois.
• Valid driver's license and access to an insured vehicle.
