Description

As a Home Care Coordinator, you'll coordinate home care services for PACE participants to help them maintain independence at home. You'll handle calls, manage schedules with contracted agencies, provide nursing services in home settings, and collaborate with the interdisciplinary team to ensure high-quality, culturally competent care.

Requirements

• Minimum 2 years of successful experience
• LVN or RN license

Responsibilities

• Coordinate home care services per RN assessment and PCP approval
• Manage home care schedules with contracted agencies
• Maintain participant medical records and conduct quality checks
• Perform physical evaluations, vital signs, and glucose monitoring in home
• Participate in interdisciplinary team meetings and care planning
