💼 About This Role

You'll lead mobile and on-premises outreach, intake, and intensive case management to help clients achieve housing stability. You'll manage a caseload of 20-25 clients and help 3-5 people per month progress from homelessness toward stability. This part-time role requires flexibility for shift work and emergency response.

🎯 What You'll Do

• Conduct outreach and connect clients to shelter, housing, and wrap-around services.
• Document and maintain case files for 20-25 active clients.
• Transport clients to appointments and arrange travel.
• Submit daily, weekly, and monthly activity and data reports.

📋 Requirements

• Bachelor’s degree in social work, public health, or 2+ years providing outreach and case management.
• Experience with de-escalation, motivational interviewing, overdose reversal, and data collection.
• Valid California driver’s license and clean DMV record.
• California Peer-to-Peer Support Specialist certification within 60 days.
