💼 About This Role

You'll lead member care as a PMHNP at Affect, providing evidence-based treatment for substance use and mental health disorders in a fully virtual environment. You'll conduct telehealth evaluations, prescribe medications, and collaborate with a care team to deliver outcomes-based care in a mission-driven, innovative culture.

🎯 What You'll Do

• Conduct initial and follow-up telehealth evaluations
• Provide care for both substance use and mental health conditions
• Prescribe medications and respond to member questions
• Complete timely documentation in EHR (Healthie)
• Collaborate with care team and attend scheduled meetings

📋 Requirements

• Active, unrestricted PMHNP license in Illinois
• Minimum 2 years of experience (addiction or mental health preferred)
• Experience prescribing Suboxone and active DEA registration
• Willingness to obtain additional state licenses
