Description

You will conduct in-home assessments and deliver evidence-based recommendations for home modifications to help older adults and individuals with disabilities live safely and independently. You'll collaborate with clients, families, and our clinical operations team, using a tech-first approach to documentation and care planning.

Requirements

• Degree in Occupational Therapy from accredited program
• Active OT license
• 2+ years clinical experience
• Reliable transportation and valid driver's license
• Proficiency with technology and digital documentation

Responsibilities

• Conduct in-home assessments and functional evaluations
• Deliver recommendations for home modifications
• Educate clients on fall prevention and safe mobility
• Collaborate with Director of Clinical Operations
• Maintain documentation through electronic system
