🎯 What You'll Do

• Perform telemedicine visits including intake H&P and follow-up care.
• Serve as provider of record and maintain a sizable active patient panel.
• Assess, diagnose, and treat adult outpatients with multiple chronic conditions.
• Coordinate care with primary care providers, specialists, and external agencies.

📋 Requirements

• Completion of an accredited graduate-level Nurse Practitioner program (Master's degree required).
• Current Nurse Practitioner licensure in Virginia and Pennsylvania.
• Ability to obtain Full/Independent Practice Authority within 6 months of appointment.
• Active national certification through the American Academy of Nurse Practitioners Certification Board (AANPCB).
