Description

You will join a compassionate hospice team providing palliative care to clients in their homes in Berks and Lehigh counties. Your work as an integral member of a multi-disciplinary health care team allows patients to remain at home with dignity and comfort.

Requirements

• Current RN license in PA
• Minimum one year recent clinical (medical/surgical) experience
• Solid computer skills; prior EMR experience preferred
• Ability to travel to cases as assigned
• Prior home care or hospice experience strongly preferred

Responsibilities

• Make home visits to clients in designated territories
• Administer medication, wound care, treatments, procedures, and comfort care
• Monitor clients' conditions and report changes to Clinical or Client Services Manager
• Execute and document doctors' orders
• Perform client assessments as necessary
