Description

You will conduct in-home assessments for older adults and individuals with disabilities, delivering evidence-based home modification recommendations to help them live independently. You'll educate clients on fall prevention, collaborate with a clinical operations team, and maintain digital documentation. This role impacts client safety and quality of life in their own homes.

Requirements

• Degree in Occupational Therapy from accredited program
• Active, unencumbered OT license
• 2+ years clinical experience
• Reliable transportation and valid driver's license
• Proficiency with technology and digital documentation

Responsibilities

• Conduct in-home assessments and functional evaluations
• Deliver personalized home modification recommendations
