You will plan, organize, and direct hospice care, focusing on community health education. Your role involves assessing patient needs, developing care plans, and coordinating with the interdisciplinary team to deliver compassionate end-of-life support.

Requirements

• Registered Nurse with experience in nursing
• Emphasis on community health education/experience
• Ability to perform complete physical assessments
• Strong communication and care coordination skills
• Knowledge of palliative nursing procedures

Responsibilities

• Complete initial and ongoing comprehensive patient assessments
• Develop and revise care plans based on nursing diagnosis and patient status
• Administer medications and treatments as prescribed
• Educate patients and families on self-care and health management
• Coordinate care with physicians and interdisciplinary team
