Description

You will provide occupational therapy services to adult and geriatric clients in their homes, performing evaluations, developing care plans, and educating patients and families to promote independent living.

Requirements

• Current North Carolina Occupational Therapist license
• Home health experience required
• Minimum one year recent work experience as an Occupational Therapist
• Graduation from an accredited occupational therapy program
• Ability to work independently and manage time effectively

Responsibilities

• Perform home visits for occupational therapy evaluations and interventions
• Assess functional status, cognitive and motor aspects, and home environment
• Develop and revise individualized intervention plans
• Identify equipment needs and environmental adaptations
• Document patient care using electronic medical records
