You'll conduct nutrition assessments and collaborate with an interdisciplinary team to develop holistic care plans for seniors. Your work directly impacts participants' health and well-being. This role stands out for its focus on integrated, person-centered care.

🎯 What You'll Do

• Conduct nutrition assessments to determine participant needs.
• Participate in IDT meetings to develop care plans.
• Deliver nutrition interventions and provide counseling.
• Oversee menu planning and meal delivery programs.

📋 Requirements

• Registered Dietitian credential.
• At least 2 years experience directing nutrition services in healthcare.
• Current BLS and First Aid certification.
• At least 1 year experience with frail or elderly population.

✨ Nice to Have

• Geriatric care or chronic disease management experience.
• Mandarin language skills.
