Description

You will be an integral member of a multi-disciplinary team providing rehabilitative care and skilled nursing to adult patients in their homes, allowing them to receive medical care while remaining at home. You'll make home visits, administer medications, perform wound care, and manage cases in Kent County, DE.

Requirements

• Current RN license in Delaware
• Minimum 1 year recent clinical (medical/surgical) experience
• Graduation from accredited nursing program
• Ability to travel to assigned cases
• Solid computer skills; EMR experience preferred

Responsibilities

• Follow designated care plan in accordance with patient needs
• Make home visits for medication administration, wound care, and procedures
• Monitor client conditions and report changes
• Perform client assessments and case management
• Accurately document observations, interventions, and evaluations
