Description

You will provide speech therapy home health visits to clients in Westford, Fitchburg, Leominster, Boxborough and surrounding towns. As part of a multi-disciplinary team, you'll conduct evaluations, develop treatment plans, educate patients and families, and document care using a tablet.

Requirements

• Current Speech-Language Pathologist license in Massachusetts
• One year of experience with adult/geriatric population
• Prior home health experience preferred
• Valid driver's license and reliable transportation
• Ability to use touch pad tablet for documentation

Responsibilities

• Perform home visits in designated geographic territories
• Conduct evaluations to assess and diagnose speech, language, communication, or swallowing difficulties
• Develop treatment plans based on evaluation, physician's orders, and goals
• Continually assess and revise care plan, collaborating with multidisciplinary team
• Educate patients and families on rehabilitative care and activities
