💼 About This Role

You'll join a multi-disciplinary health care team providing rehabilitative care and skilled nursing to adult and geriatric clients in their homes across Berks County. Your work enables clients to remain independent while receiving necessary medical care. All new grads receive additional support during orientation.

🎯 What You'll Do

• Perform diagnostic tests and measurements for mobility and strength
• Develop and implement individualized care plans
• Educate clients and families on rehabilitative care
• Document observations and interventions using a touch pad tablet

📋 Requirements

• Current Pennsylvania Physical Therapist license
• Graduation from an APTA-approved program
• Ability to work independently and manage time effectively
• Solid computer skills; prior EMR experience preferred
