💼 About This Role

You'll help infants, children, and adults transition from hospital or HICU® services to home care in Philadelphia. You'll partner with CHOP to care for clients with rare diseases and train the ongoing care team. This role offers a unique opportunity to work with complex pediatric patients in a home setting.

🎯 What You'll Do

• Deliver quality care to clients with complex conditions
• Provide extensive training to ongoing care team
• Support transition of care from hospital to home

📋 Requirements

• Current Registered Nurse license
• NICU, PICU, ICU, or home care trach/vent experience required
• Current CPR certification
• Ability to travel throughout Philadelphia County

• High-tech adult and pediatric simulation passing score
• Experience in teaching or training settings
