🎯 What You'll Do

• Complete Start of Care admissions and OASIS assessments
• Conduct follow-up and evaluation visits in designated territory
• Monitor client conditions and report changes to clinical team
• Document patient care interventions and evaluations accurately

📋 Requirements

• Current and unrestricted CA RN license
• Current BLS/CPR Certification
• Valid CA Driver's License
• Experience conducting assessments and Start of Care OASIS
