Description

You will provide physical therapy services to adult and geriatric clients in their homes, helping them regain independence and improve quality of life. As part of a multidisciplinary team, you will develop care plans, perform assessments, and educate clients and families.

Requirements

• Licensed Physical Therapist
• One year prior clinical experience
• Ability to perform home health visits
• Experience with adult and geriatric clients
• Valid driver's license and reliable transportation

Responsibilities

• Make home visits to clients in designated territories
• Perform diagnostic tests and measurements including mobility, strength, and balance
• Implement interventions for wound prevention and repair
• Develop and implement individualized care plans
• Educate clients and family members on rehabilitative care
