💼 About This Role

You'll join Vynca's Enhanced Care Management team in Los Angeles, CA to serve as the primary point of contact for clients with complex needs, coordinating care across providers. You'll assess member needs, develop care plans, and advocate for clients to ensure access to appropriate services. This hybrid role requires travel throughout Los Angeles County.

🎯 What You'll Do

• Assess member needs in physical health, mental health, SUD, and social supports
• Oversee development of client care plans and goal setting
• Conduct outreach and engagement activities for ECM program linkage
• Complete documentation and maintain patient health records in EMR system

📋 Requirements

• Active, unrestricted California LVN license
• 2+ years of experience as a care manager or care navigator supporting vulnerable populations
• Working knowledge of government and community resources related to social determinants of health
• Valid driver's license, clean driving record, and reliable transportation

✨ Nice to Have

• Bilingual (English/Spanish)
• Experience with Motivational Interviewing, Harm Reduction, or Trauma-Informed Care
• Proficiency in Google Workspace and MS Office
