💼 About This Role

You'll provide supervisory and clinical oversight for Long-Term Care and Skilled Nursing Facilities across rural Southeast Colorado. Your work will directly support underserved senior communities with a flexible, physician-led approach. This role offers extremely flexible scheduling with minimum in-person rounding once per month.

🎯 What You'll Do

• Provide supervisory oversight and monthly in-person rounding
• Manage chronic and complex geriatric patients in LTC/SNF settings
• Collaborate with facility nursing and administrative teams
• Document care in EMR in a timely manner

📋 Requirements

• MD or DO degree
• Board Certified/Eligible in Internal Medicine, Family Medicine, or Geriatrics
• Active CO medical license (or ability to obtain)
• DEA license

• Experience in LTC, SNF, geriatrics, post-acute care, or rural medicine
• Familiarity with Medicare and value-based care
