Description

You will deliver in-home speech therapy to adult and geriatric patients, helping them communicate clearly and participate in everyday life. This role offers flexible scheduling, consistent patient volume, and supportive team collaboration.

Requirements

• Active Speech Language Pathologist (CCC SLP) license in good standing
• BLS CPR certification
• Reliable transportation
• Basic comfort with technology (email, texting, EMR systems)
• Ability to see patients in their homes

Responsibilities

• Deliver in-home speech therapy services to adult and geriatric patients
• Evaluate communication, cognition, and swallowing to develop treatment goals
• Implement rehabilitation and therapeutic exercise programs
• Educate patients and families on recovery plans and preventive care
• Maintain accurate home health care documentation per Medicare standards
