Description

As a home health Physical Therapist, you'll perform home visits for adult and geriatric clients, conducting diagnostic tests, developing individualized care plans, and educating clients and families to promote health and independent living.

Requirements

• Current VA Physical Therapist license
• Graduation from an APTA or CAAHEP accredited program
• Ability to work independently
• Strong interpersonal and problem-solving skills
• Solid computer skills; EMR experience preferred

Responsibilities

• Make home visits to clients in designated territories
• Perform diagnostic tests and measurements (mobility, range of motion, balance, etc.)
• Implement wound prevention and repair interventions
• Develop and implement individualized care plans with manual therapy, gait training, balance
• Educate clients and families on rehabilitative care
