Description
As a Community Health Worker, you will support patients and families as part of an interdisciplinary care team in a primary care setting. You'll assess social determinants of health, coordinate with community organizations, and empower patients through motivational interviewing and goal setting.
Requirements
- CHW State course completion.
- Bachelor's degree in health-related field, social work, or health care administration (preferred).
- Background in managing populations with complex medical and behavioral needs.
- Experience in community health or public health setting.
- Proven knowledge of Medicaid population.
- Experience documenting in an electronic health record.
- Ability to organize and manage a caseload and assist with patient navigation.
- Full COVID-19 vaccination required.
- Proficiency with computer skills (Microsoft Word, Excel, Access, PowerPoint, web-based applications).
- Demonstrated ability to work independently and collaboratively.
- Excellent interpersonal, verbal, and written communication skills.
- Bilingual Spanish speaking a strong asset.
- Driver's License.
- High School Diploma/GED (Bachelor's preferred).
Responsibilities
- Complete initial assessments with patient/family to identify strengths and barriers to care.
- Build trusting relationships through rapport building.
- Empower patients via motivational interviewing and goal setting.
- Coordinate with interdisciplinary care team, family, and healthcare providers.
- Advocate for patients to ensure needs are represented and supported.
- Document in EPIC and native EMR.
- Assist with setting up social services: food assistance, housing transitions, state/federal applications, insurance, benefits, long-term care, child/family resources.
- Understand and address social determinants of health.
- Collaborate with Behavioral Health Clinicians to improve patient outcomes.
