Description

You will provide occupational therapy services to adult and geriatric patients in their homes, collaborating with a multidisciplinary team to help clients achieve independence. Your role includes evaluations, care planning, and patient education.

Requirements

• Current North Carolina Occupational Therapist license
• Minimum one year of recent work experience as an Occupational Therapist
• Graduation from an accredited occupational therapy program
• Ability to work independently and manage time effectively
• Strong interpersonal, organizational, and problem-solving skills

Responsibilities

• Perform home visits to clients in designated geographic territories
• Conduct thorough evaluations of functional status, including muscle function, endurance, and ADL/IADL performance
• Develop individualized intervention plans and assess home environment for hazards
• Identify equipment needs and environmental adaptations
• Educate patients and families on rehabilitative care activities
• Document observations and interventions using a touch pad tablet
