🎯 What You'll Do

• Engage and build trust with members in the community.
• Achieve member enrollment and engagement targets daily.
• Help members create personal action plans and track progress.
• Deliver physical health coaching as needed.

📋 Requirements

• 2+ years of experience with serious mental illness in community settings.
• Experience working in Philadelphia communities.
• Valid driver's license.
• High school diploma or equivalent.

✨ Nice to Have

• Spanish fluency or other languages.
• Certified Peer Specialist Training.
• Experience with Substance Abuse Disorders.
