Description

You will lead the Care Coordination team for CookUnity's Medically Tailored Meals programs, ensuring seamless member onboarding, order support, and compliance with payer requirements. You'll translate healthcare contracts into operational workflows, monitor team performance, and coordinate cross-functionally to minimize errors and optimize reimbursement.

Requirements

• Experience managing operations in a regulated healthcare environment
• Experience leading a Care Coordination or similar team
• Ability to translate healthcare contract requirements into scalable workflows
• Strong cross-functional coordination skills (with Legal, Compliance, Finance, Product)
• Data-driven approach to monitor and improve operational metrics

Responsibilities

• Manage and lead the Care Coordination team (hiring, coaching, performance reviews)
• Define and enforce operational workflows for intake, eligibility verification, and order support
• Monitor team metrics: intake accuracy, QA scores, SLA adherence, member satisfaction
• Act as escalation point for complex member issues
• Lead operational onboarding of new healthcare partners and translate contract requirements into processes
