🎯 What You'll Do

• Conduct home visits and community outreach to engage patients.
• Assess patient needs and develop health goals collaboratively.
• Coordinate care with interdisciplinary teams and community resources.
• Provide chronic care coordination and support for high-complexity cases.

📋 Requirements

• 2+ years experience as a Community Health Worker, Peer Support Specialist, or Medical Assistant.
• 2+ years pediatric experience in care coordination and referrals.
• Valid driver's license and auto liability insurance.
• Proficiency in EMR systems and data entry.

• Multi-lingual capabilities.
• Prior home care or Enhanced Care Management experience.
