You'll join a multi-disciplinary home health care team providing rehabilitative care and skilled nursing to clients in the West Valley area. 🎯 What You'll Do

• Make home visits to clients in designated geographic territories.
• Perform diagnostic tests and measurements for mobility, strength, and balance.
• Develop and implement individualized care plans including therapeutic exercises.
• Educate clients and family members on rehabilitative care activities.

📋 Requirements

• Current AZ Physical Therapist license.
• Minimum one year of recent work experience as a Physical Therapist.
• Graduation from an APTA-approved Physical Therapy program.
