You'll provide skilled occupational therapy services to individuals in their homes, helping them live safely and independently. You'll serve as an educator, patient advocate, and member of the interdisciplinary care team.

🎯 What You'll Do

• Evaluate patient needs in home environment
• Develop and implement occupational therapy plans
• Educate patients and families on home safety
• Collaborate with interdisciplinary care team

📋 Requirements

• Current unrestricted CA Occupational Therapist License
• Current BLS/CPR Certification
• Valid CA Driver's License
• Knowledge of local community resources

• Experience in home health care highly preferred
• Experience with high acuity patients
• Case management fundamentals highly desired
