Description

You will play a critical role in Pair Team's whole-person, interdisciplinary care model, directly outreaching and engaging individuals with mental illness, substance use disorder, or homelessness. Most duties are remote, with 2-3 on-site community visits per month near Atwater.

Requirements

• Bilingual Spanish speaking
• Highly motivated and empathetic
• Experience with underserved populations, serious mental illness, or substance use disorder
• Willingness to travel for community visits 2-3 times per month

Responsibilities

• Conduct outreach and engage individuals with serious mental illness, substance use disorder, homelessness, or high medical needs
• Deliver whole-person care including clinical, behavioral, and social support
• Collaborate with interdisciplinary care team (nurses, social workers, community health workers)
• Coordinate timely, personalized support using AI and automation to reduce admin burden
• Participate in 2-3 monthly on-site community visits
