Description

You will perform outpatient therapy evaluations and develop care plans for adult and geriatric patients in senior living communities or group homes. Working as part of a multi-disciplinary team, you will use evidence-based approaches to maximize functional independence and coordinate care transitions from home health to outpatient services.

Requirements

• Current North Carolina Occupational Therapist license
• Minimum one year recent work experience as an Occupational Therapist
• Graduation from an accredited occupational therapy program
• Ability to work independently and manage time effectively
• Strong interpersonal, organizational, and problem-solving skills

Responsibilities

• Perform outpatient therapy evaluations to develop patient care plans
• Utilize current research-based approaches to maximize functional independence
• Coordinate care transitions from home health to outpatient services
• Educate clients, family, and caregivers on care plans
• Conduct evaluations, treatments, and discharge planning using EMR platform
