You'll combine clinical excellence with inspiring leadership in a value-based primary care practice revolutionizing senior healthcare. You'll deliver care directly to seniors in their homes while leading a team of 6-8 providers, balancing patient care with management responsibilities.

🎯 What You'll Do

• Conduct comprehensive Welcome Visits and personalized Care Plans
• Provide clinical coverage during provider absences
• Lead and develop 6-8 Nurse Practitioners and Physician Assistants
• Optimize provider schedules and drive quality improvement initiatives

📋 Requirements

• Active CA APRN license with unrestricted DEA schedules 2-5
• Master's degree in Nursing with Family or Adult-Gerontology specialization
• Minimum 3 years specialized geriatric experience
• Minimum 3 years people management supervising clinical staff
